A Collaborative Care Practitioner listens first and prepares you. Your Effica physician then assesses you, discusses your options and decides the right medical care with you.

How to get referred

Take our referral form to an authorized referring practitioner.

Download the fillable Effica referral form, bring or send it to a practitioner who is authorized to refer in your province, and they will complete and fax it to our clinic. You do not need a confirmed diagnosis before being referred.

Every Effica pathway follows the LIGHT™ Method. Here is how each stage looks for this pathway, from your first intake to what happens after your plan is in place.

L

Step — Listen

It begins with your referral and your story.

Once your referral is received, we start by understanding what you are experiencing rather than asking you to compress it into a short appointment.

What happens
  • Your referring practitioner sends the Effica referral form to our clinic.
  • We confirm receipt and send you a secure, comprehensive intake covering your symptoms, health history, medications and what matters most to you.
  • You complete the intake in your own time, before anyone asks you to explain yourself in a rushed visit.
How long it takes
  • Most patients complete their intake in 20 to 30 minutes, and it does not need to be finished in one sitting.

Step — Illuminate

Your information is connected before your appointment.

Clinical Intelligence™ brings your intake together so your care team sees patterns, connections and areas that deserve attention before you meet.

What happens
  • Your symptoms are organized by pattern, severity and impact on daily life.
  • Relevant history, medications and risk factors are surfaced for your clinicians.
  • You receive Intake Insights so you can see what we have understood and correct anything before your visit.
Behind the scenes
  • Clinical Intelligence™ supports your clinicians. It never replaces clinical judgment or makes decisions about your care.

Step — Guide

A Collaborative Care Practitioner, then your physician.

Your care is delivered by two people who already know your story before you speak.

What happens
  • You meet with a Collaborative Care Practitioner, a registered nurse, nurse practitioner or clinical pharmacist with menopause expertise, who hears your story, fills gaps and provides education.
  • You then have a focused virtual consultation with an Effica physician centred on menopause assessment and treatment.
  • Your physician discusses evidence-based options with you, including menopausal hormone therapy and non-hormonal approaches, and you decide together.
How long it takes
  • Appointments are virtual. You do not need to travel, and you can attend from home.

Step — Heal

Treatment, investigations and follow-up.

Heal is where your plan becomes care, with medical follow-up based on clinical need.

What happens
  • Prescriptions, laboratory testing, imaging and referrals are arranged when clinically appropriate.
  • Medically necessary physician follow-up is provided as your treatment is adjusted.
  • An assessment summary and plan are shared back with your referring physician with your consent.

T

Step — Thrive

A clear transition, and more if you want it.

When your symptoms are managed, care returns to your primary care provider with everything they need to continue.

What happens
  • You transition back to primary care with a documented plan.
  • If you want a broader look at prevention, risk, screening and how you want to age, our non-insured Personalized Healthspan Consult and Membership are available. Neither is required, and neither provides faster access to insured physician care.
